II. TERMS OF TRADE
Motion of exchange the residence card should be submitted within 14 days after the occurrence of the premises for replacement, i.e.
- Data change
- Change of the owner’s face image in comparison to the face image in the residence card to a degree that makes identification of the card’s owner difficult
- decline; - damages
Motion of exchange the residence card should be submitted at least 30 days before the expiry of the residence card.
III. REQUIRED DOCUMENTS
A copy of the completed application for replacement of the residence card;
- undamaged, color, good quality;
- 35 mm x 45 mm;
- Made not earlier than 6 months before the date of submitting the application;
- Showing the image of the foreigner's face from the top of their head to the top of their shoulders so the face could take up 70-80% of photography space, with the bright, consistent background, in frontal position, looking straight ahead with the eyes open, unobstructed hair, with the natural facial
expression and closed mouth and also with the natural-looking color of the skin;
Attention! Foreigners who wear headwear according to their Articles of Faith, can attach a photo of them wearing their headdress to the motion as long as the face image is fully apparent. In this case the foreigner must attach a declaration of belonging to communion.
A copy of the travel document (the original one should be available for inspection)
Documents justifying the replacement of the residence card, for example: in case of changing the residential address, the current confirmation of registration must be attached to the motion;
Proof of payment of the fee for issuing/replacing the residence card.
- 50 zł - card issuance
- 100 zł - in case of the first culpable loss/damage of the card;
- 150 zł - in case of every next culpable loss/damage of the card;
V. RELIEF IN PAYMENT — 50% appertains to the foreigners:
- who are in a difficult material standing;
- whose purpose of stay is to study in upper secondary schools and academic institutions in Poland;
- minors who were under the age of 16 on the day of submitting the application for issuing or replacing the card of residence.
Documents requested to obtain a fee reduction:
- a certificate from the own social welfare body on the use of social service cash benefits at a given time;
- a certificate which confirms studying in upper secondary school or academic institution at a given time;
- birth certificate of the minor foreigner and in case of inability to obtain it - travel document or another identity document which confirms the age of the minor foreigner.

The fee is payable during the submission of an application of exchange the residence card at the cash desk of the authority or to a bank account.
VI. DEADLINE FOR DEALING THE CASE
Collection of the card takes about 2-4 weeks
The application for the issue/replacement of the card must be submitted in person - fingerprints are taking from the foreigners who are 6 years old or older
During its validity period, the residence card confirms the identity of the foreigner during their stay in Poland and gives them authority, along with the travel document, to multiple border crossings — no visa required.
The originals or copies of the above documents must be provided if their compatibility with the original was certified by the notary or by the party's attorney who acts in the case as a counsel, legal counsel, patent agent or tax preparer (art. 76 a kpa).
Photocopies of documents can not be evidence in administrative procedure.
The foreigner is obliged to collect the residence card in person, in case of minors under the age of 13 — it is collected by its statutory representative or curator.
In case of loss or damage of the document, the foreigner must announce it to the organ which issued the document in 3 days from the loss/damage. Foreigners will get a certificate confirming this fact for free. The notification is made on the form.

INVITATION for Foreigner - discharge report*
The inviter may get an entry for the record for the invited foreigner as a document which confirm possession of resources which are sufficient to cover costs of the due stay in Poland, including cost of accommodation and maintenance, return trip to the country of origin or accomodation and also transit to the third country which will allow the exit.
The inviter can be:
- citizen of Poland who is living in Poland, citizen of other country of the EU, Norway, Iceland, Liechtenstein or Switzerland or the member of their family who reside in Poland and can legally live here;
- foreigner who reside in Poland legally for at least 5 years directly before write the invitation out or foreigner who has permit to constant stay or permit to stay of long-term resident of EU;
- corporate entity or business unit with no legal personality which have their residence in Poland.

V. TIME TO SETTLE THE MATTER — one month at the maximum
VI. NECESSARY DOCUMENTS
The filled form of the motion of the invitation entry for the record Attention! Filling the form incompletely will cause the monition of the foreigner to fill it completely in 14 days from the delivery under penalty of leaving the form without examination.

Attention! By including undermentioned documents to the motion, you may limit the amount of the official correspondence and shorten the time to handle this case.
Documents which confirm possibility of covering the cost of foreigner’s residence, including: accommodation, maintenance, costs of the travel back to the country of origin or the country of residence or the transit to the third country which will allow the entrance (details below);
Documents which confirm legal claim to the residential premises if it’s the inviter’s residential premises and it was designated as the travel arrangement of the foreigner or document which confirms the ability to provide them accommodation in a different place.
Specific establishment of the resources which the inviter should have:
The inviter should have resources to cover the cost of the foreigner’s stay in Poland, including accommodation and maintenance for them, every member of their family who are maintained by them and every invited foreigner, at least total of 515 zł for every month of foreigner’s stay for every each person (these documents must be furnished, for example, as a bank statement).
If the period of the foreigner’s stay is shorter than the month, the inviter should have resources to cover the cost of the foreigner’s stay in Poland - the amount from the point 1, downgrade proportionately to the amount of days of residence.
If the inviter as an individual does not possess the resources which are mentioned in a part one, for the whole period of the foreigner’s stay in Poland the duty of having this resources is considered to be achieved if the inviter has resources mentioned in point one every month of the foreigner’s planned stay, which are destined for them, every member of their family who are maintained by them and every invited foreigner (you must show the documents which confirm regular and constant getting of the resources, for example employer's certificate about the the amount of income).
When invited foreigner is descendant, ancestor or a spouse of the inviter, some of the parents, siblings or children of the inviter’s spouse, the inviter should possess resources to cover the cost of the foreigner’s stay in Poland in the amount of at least 200 zł for every month of foreigner’s planned stay
or commands this amount of money in every month of foreigner’s stay (you must present the same documents as in the point 1 or 3)
The inviter should also possess resources to cover the cost of the return trip of the invited foreigner to their country of origin or residence or the cost of the transit to the third country which will allow the entrance, in the amount which is equivalent of the ticket to this country but also not less than:
200 zł - return trip or transit to the neighbour country of Poland
500 zł - return trip or transit to the European country different than countries described in the above point
2500 zł - return trip or transit to the non-European country
Duty of possession of the resources to cover the cost of the return trip of the invited foreigner to their country of origin or residence is considered to be fulfilled if the inviter demonstrates that the foreigner possesses a return ticket to the country of their origin or residence.
Attention! If it is necessary to explain or clarify the evidence of the case during the proceedings, the foreigner can be ordered out to provide some other documents or to make a statement which will confirm the circumstances which are described in the motion. In case of hospitalisation of the
foreigner, voivod may appoint a doctor to give an opinion if the foreigner’s health state forbids them leaving government expenditure of Poland.

National Trail against Human Trafficking
The National Trail against Human Trafficking is a basic document about the human trafficking combat and prevention system in Poland. The National Trial is a governmental document accepted by the Council of Ministers, which obligate its addressees to handle particular tasks regarding combating
the human trafficking. The addressees of aforementioned tasks are respective individuals of central administration and other institutions and non-governmental organization which are voluntarily concerned in actions against the human trafficking in Poland. Tasks which are referred in National
Trail against Human Trafficking are government-funded.
The first document like that, “The National Trail of Combat and Prevention of Human Trafficking” was accepted by Council of Ministers on the 16th of September 2003. Currently the effective trail is “The National Trail against Human Trafficking for 2020-2021”, which was accepted by the Council of Ministers on the 10th April 2020.
The Provincial Prevention of Human Trafficking Team instituted by the Warmińsko-Mazurski voivod
The Warmińsko-Mazurski voivod, while accomplishing a task of coordination within the system of social service, actions regarding preventing of human trafficking and also support for the victims of this crime, also having regard to tasks determined in “The National Trail Against Human Trafficking”, instituted The Provincial Prevention of Human Trafficking Team in 2015.
This team consists of representatives of the entities responsible for combating, prevention and help for the victims of this crime, i.e. representatives of: Provincial Police Office in Olsztyn, Central Bureau of Investigation of the National Police Headquarters in Olsztyn, Warmińsko-Mazurski Border Guard Unit, Warmińsko-Mazurski Customs and Tax Office in Olsztyn, District Prosecutor's Office in Olsztyn, Circuit Court in Olsztyn, District Labor Inspectorate in Olsztyn, Provincial Employment Bureau in Olsztyn, local councils, i.e. Association of Warmińsko-Mazurskich Communes and The
Convention of the Poviats of the Warmińsko-Mazurskie province, Marshal's Office of the Warmińsko-Mazurskie province, incl. Regional Center of Social Policy, Local Education Authority in Olsztyn, non-governmental organization, i.e. Elbląg Association of Social Welfare Organizers and Elbląg Center for Mediation and Social Activation, University of Warmia and Mazury and Warmińsko-Mazurski Province Governor's Office.
The goal of this team is to create an information exchange platform and the cooperation between entities concerned in counteracting the human trafficking and rationalization of the preventive activity, support and reintegration of the victims of the human trafficking and also animation of the
local governments and non-governmental organizations in this case.
In 2019 the members of the team, within the activity of their entities, were taking action in the interest of counteracting the human trafficking, including:
prevention actions which includes organisation of the events which promote issues of human trafficking, directed to the general public, including information campaign, which was implemented in the following way:
— they took a part in meetings with parents and residents during which they were talking about human trafficking and spread workpack about this phenomen (bills and posters)
— they arranged film shows about human trafficking
— they were running an information campaign about forced labour directed to the citizens of Poland which depart to work abroad
— they disseminate workpack about the human trafficking phenomen in public locations as employment bureaus, municipal and district offices, community and health centres, tourist points, courts or border crossings
— they were talking about the human trafficking phenomen in media
— they added information about human trafficking on the websites of their institutions
— they took a part in conferences during which they were talking about topics connected with the prevention of the human trafficking, popularization of the knowledge about human trafficking among teenagers, students and in attendance centres by arranging informational meeting.
During the meetings with the teenagers, they were conducting learning activities in the aforementioned field, they were presenting materials relevant to foreign exits, taking up a job and making new friends - in terms of risks connecting with the human trafficking and also showing the
movies about this phenomen and presenting information about institutions which help the victims of human trafficking, promoting legal and safe exits to work abroad among people which are looking for a job and the employers. What is more, they organised “Job Fair” during which they were distributing workpack, running workshops for people who are looking for a job, and also giving information about
Directly before the holiday they informed about legal and safe exits for work in EOG countries, they put the information (in Polish, English, Ukrainian and Russian) for foreigners from Third World countries about taking up a job in Poland legally and also for Polish employers who
are interested in hiring foreigners on a website wupolsztyn.praca.gov.pl, they organised exposition of mobile exhibition “Countenances of Human Trafficking” in facilities in Olsztyn, which goal was to rise the social awareness about risks of human trafficking and also they organised the conferation which was called “Modern challenges of the labour market - hiring the foreigners” as an answer for the needs of entities conducting business activity in the area of Warmińsko-Mazurskie province, during which they were talking about human trafficking in the context of employment.
